OAB posted a very thorough overview on site.
When you send in your request to roll a team (and pay the fee), you will receive the warrior profiles in the mail - just like normal with an arena. The profile, as normal, has the warrior ID.

The ID's (arena #) are different for FTF and Mail-In - usually arenas 98 and 99.
